Cantonese is never written in Simplified Chinese form. It is only written with Traditional Characters.
This disc has 2 sets of Simplified Chinese subtitles? Are they labeled any differently?

The back cover lists the subtitles both in English and Chinese (with x/y/a/b representing Chinese characters):
Traditional Chinese (Cantonese/Mandarin) x (a/b)
Simplified Chinese (Cantonese/Mandarin) y (a/b)
English
In the subtitle menu there are four differently titled Chinese subtitles for selection in addition to English but these are only listed in Chinese. The same letters are used as in the back cover. Mapping them to the English words from x (a(b) etc I would get:

Simplified Chinese (Cantonese)

Simplified Chinese (Mandarin)

A possibility is that the two labeled "Cantonese" are subtitles for the Cantonese audio and the "Mandarin" ones are subtitles to caption the Mandarin dialogue.
This seems to be the only logical explanation because there literally is no way to write Cantonese in a simplified form. I will add it as what I wrote above.
