I could only find packaging pictures for the Zavvi Steelbook and it has both discs inside one case.

My mistake, it's a bundle of the Chapters 1 & 2 and I meant to say that each film is in its own case - with a UHD BD and regular BD inside. They appear to just be the standard retail versions of each film in a slipcover, complete with the barcodes on the individual sleeves.
The commentary is on both discs, but all of the other extras are on the standard BD only. The metadata on the discs date the regular BD to 2015 and the 4K disc to 2017.
My apologies for making this much more confusing than it needed to be!
