This is now OOP, and was only available through http://necrophagia.com/idrink.html. Limited copies came with autographs of Durston, Bhaskar, Kierney, and Damon (scribed strategically on a photo layout printed on the backside of the cover), and Lynn Lowry's autograph was included on a separate postcard.
